NOTE: It is important that you ensure that your child has the relevant medication they may need with them when attending matches, training or any HTJFC event. It must be clearly labelled with their name. If your child is asthmatic then please bring a spare inhaler for their coach to keep in case of emergencies.

Please read the following declarations and CHECK the box to the left of each one to indicate that you understand the content. Then please CHECK the box to confirm all your details in the box provided.
MEDICAL CONSENT*: The club will make every effort to contact parents/guardians in the event of any illness or accident. However, I have completed the medical details above and consent that in the event of such an incident any necessary treatment can be administered to my child by suitably qualified persons. I understand that whilst team and club officials will take every reasonable precaution to ensure that accidents do not happen, they cannot necessarily be held responsible for any loss of or damage to property or injury suffered by my child.
MEDIA CONSENT: The club may wish to take photos or videos of the team or individuals in it for the promotion and celebration of the activities of the club and for training purposes. I give permission for photographs/videos to be taken of my child whilst participating in football for Horbury Town JFC, and, that such photographs/videos can be used for the promotion of the club, including, but not limited to, publication on social media, in match reports & newsletters. I further agree to my child's name to be included in such media coverage where applicable.
THIRD PARTY*: I give permission for the club to pass on my contact details to third parties associated with the Junior Football Club (e.g. County FA, Leagues etc).
COMMUNICATION: (Ages 16+): Coaches may wish to communicate directly with your child via a group text/email/chat group to discuss specific club related activities e.g. informing players of match arrangements, changes to training, venue changes etc. I consent to my child being included in these groups and understand that all communications are overseen by the Club Welfare Officer.

I have read the Club's Code of Conduct for parents and spectators, I understand my obligation towards the team, the opposing team, match officials and to the Club, and I agree to abide by its terms at all times.*
I have read the Respect Code of Conduct for Players with the player named on the registration form and he/she understands his/her obligation towards the game and the team, and will treat with respect the laws of the game, the opposing team, the match officials and to the officials and spectators of both teams, and I agree to abide by its terms at all times.*
I agree to be bound by, and to observe, the Club's Rules, Constitution and Policies (including GDPR), the Rules and Regulations of The Football Association and relevant Leagues, and all competitions in which the club participates. I confirm that the above information can be used by officials of Horbury Town JFC in the running of the junior football club.*

SUBSCRIPTIONS

Subscriptions for the year are £175. (Sibling discount £165 for 2nd child, £155 for 3rd child).

I agree to pay the full amount owing before the start of the season, and am aware of the No Play Until You Pay Policy in place (if full subs are not paid your child will NOT be insured).*

I understand it is a requirement for a responsible Adult (Aged 18+) to remain onsite throughout the training session each week and at matches, and I agree that a responsible Adult (Aged 18+) will be present. I further agree to my child's details to be entered on to the FA Whole Game System.
